What the assessment is for
The purpose is classification, not recovery. From the wallet type, the failure mode and the surviving information, the remaining search space can be estimated well enough to say whether recovery is likely, borderline or impossible.
Some cases turn out not to need recovery work at all: a wrong derivation path, a hidden wallet behind a passphrase, an unscanned account index, or a keyboard-layout mismatch. Those answers cost you nothing.
Before you fill in the form
- Stop experimenting on the original wallet data and copy it to separate storage.
- Do not reinstall wallet software or reset a browser profile.
- Write down, offline, whatever you remember about the password or the missing words.
- Locate a public address from the wallet — verification depends on it.
- Note the wallet software and roughly when the wallet was created.
